All of the policies regarding the use of technology provided by Simmons College of Kentucky shall apply to the usage of the Mobile Hotspot Modem assigned to the employee named in this agreement.

The policies for returning property owned by the school shall apply to Mobile Hotspot Modem usage as deemed necessary for the sole purpose of staff accountability and liability in case of misuse.

I am being allowed the temporary use of the above-mentioned mobile hotspot modem (“device”) to assist me in my work at Simmons College of Kentucky.

This device is the property of Simmons College of Kentucky.

I am responsible to return this device in good working condition at the option of the college administration or
when my employment with the college should end.

The device can only be returned directly to the Library, and I may not just leave it with any employee on campus.

I must receive a receipt at the time of its return.

I agree to keep this device solely in my possession and to use it solely for furthering my work for the college.

There is not sufficient bandwidth on this device for private gaming or non-educational use.

I will not try to alter the software pre-installed on this device.

The college is not responsible for any lost data or material that I have prepared via this device, should the signal be lost or the device ceases to work.

It is my responsibility to back up my data for safe-keeping as I go along.

If I lose, damage, or fail to return the device according to the terms of this agreement, the price of its replacement may be deducted and offset against any payroll moneys due to me.

The current estimated value of this device is $100.

I will not be charged for reasonable wear and tear or for device malfunctions so long as I have otherwise complied with the terms of this agreement.
